Tired all the time? The blood values that can explain why

Tiredness is one of the most common reasons to take a blood test

Everyone is tired sometimes. But when the tiredness will not lift despite sleep, when weeks pass and the energy does not come back, most people start to wonder whether something in the body is behind it. That is a reasonable question, and part of it can be answered with a blood test.

A blood test cannot measure tiredness. What it can do is show or rule out several of the most common physical explanations: iron deficiency, a thyroid working too slowly, low levels of certain vitamins, and blood sugar that sits wrong. Each of these is common, each can be acted on, and none of them shows on the outside.

A normal result is also an answer. It lets you turn with more confidence to what most often lies behind long-term tiredness: sleep, stress and the load life is putting on you. This guide walks through what the different blood values can show, roughly in the order it usually makes sense to think about them.

Iron: the most common explanation that shows in blood

Iron deficiency is one of the most common physical causes of long-term tiredness, especially in women who menstruate. The body uses iron to build hemoglobin, the protein that carries oxygen in the blood. When iron runs short, muscles and brain get less oxygen than they need, and it is felt: low stamina, breathlessness on stairs, pallor, sometimes headaches and trouble concentrating.

The most important value is ferritin (a measure of the body's iron stores). The stores can be low long before hemoglobin, the classic blood count value, starts to fall. Many people feel tired already in that range, although research is not fully settled on exactly where the line goes. Ferritin also has a quirk: it rises with inflammation, so a normal ferritin does not always rule out iron deficiency. That is why transferrin saturation (the share of iron's transport protein actually carrying iron) is often measured alongside, to help read the picture.

A blood count belongs to the same investigation. Hemoglobin shows whether the deficiency has reached the point of anemia, and MCV (the size of the red blood cells) hints at how long it has been going on, since the cells grow smaller as iron runs out. Two things are worth knowing: iron supplements taken on your own before the test can mask the picture, and a low iron result in someone who does not menstruate should always be followed up by a doctor, because the cause then needs looking into.

The thyroid: the body's pace

The thyroid governs metabolism, the pace of almost everything the body does. When it works too slowly, which is called hypothyroidism (an underactive thyroid), the whole system slows down. The tiredness of an underactive thyroid usually has company: feeling cold, dry skin, constipation, weight gain without changed habits, low mood and sometimes hair loss.

Three values give a good picture. TSH (the pituitary's control signal to the thyroid) rises when the brain tries to push a gland that is delivering too little. Free T4 and free T3 (the two active thyroid hormones) show what the gland is actually producing. The combination says more than TSH alone, since the control signal can sit within range while the hormones have started to drift.

An underactive thyroid is more common in women and becomes more common with age. It often develops slowly, over years, so many people have time to get used to the tiredness before the thyroid crosses their mind. A value outside the range does not automatically mean disease, but it is a clear reason to see a doctor, who can judge whether treatment is needed.

The vitamins: D, B12 and folate

Three vitamins come up again and again when tiredness is investigated. Vitamin D is made in the skin from sunlight, and at Swedish latitudes the winter months do not offer much of it. Low levels are common and have been linked to tiredness, low mood and aches, although research on how much supplements help against tiredness specifically is mixed.

B12 is needed for the nervous system and for making red blood cells. Deficiency can bring tiredness, tingling or numbness in hands and feet, memory problems and anemia. The risk is higher for anyone who eats little or no meat, has an inflammatory bowel disease, or takes certain medicines, for example metformin or acid-suppressing medication over a long time.

Folate works closely with B12 in blood formation, and a deficiency looks similar. Both are easily measured in the same draw. The same principle as for iron applies here: supplements taken on spec before testing can hide a deficiency that would have been worth knowing about, so measure first and supplement after.

Blood sugar, inflammation, and what blood cannot show

Blood sugar that sits wrong often shows up as energy dips: heavy tiredness after meals, difficulty holding weight steady, thirst. HbA1c (long-term blood sugar, an average over the past two to three months) shows where the sugar has been over time, and fasting glucose where it is right now. Raised values in the quiet range before diabetes are common and often respond to changes in habits, which makes them worth catching early.

hs-CRP (low-grade inflammation) can show whether the body is carrying a silent inflammation, which sometimes goes together with tiredness. It is a broad marker without a single explanation, and a raised value is mainly a reason to let a doctor look at the whole picture.

Then the honest part: the most common causes of long-term tiredness do not show in any blood test. Lack of sleep, sleep apnea, stress, depression and burnout account for a large share of all tiredness, and the blood test's role is then to rule out the physical causes so the right help can be found. A test where everything is normal still does its job: it moves the search in the right direction.

How to approach testing, and when to see a doctor

A sensible first choice depends on your situation. If you menstruate, the iron values are statistically the most likely explanation and a natural place to start. If you feel cold easily, have dry skin or have noticed weight gain, that points more toward the thyroid. If you eat mostly plants, or take acid-suppressing medication, B12 and folate belong on the list. If you would rather take one broad look, all of these tracks fit in a larger health check in a single draw.

Two practical points. Take the test before you start any supplements, so the result shows your true level. And keep your results: values followed over time say more than any single one.

Some things should not wait for a blood test. Seek care directly if tiredness comes with unintended weight loss, night sweats, blood in the stool, breathlessness at rest or chest pain. And whatever a result shows, the same rule holds: values outside the range are a question for a doctor, who reads them in the light of your history and symptoms. A blood test never makes a diagnosis, but it can show where it is worth looking.
